Dosing evidence

Dose at a glance

There is no FDA-approved dosing. Pemvidutide is not FDA approved, so there is no FDA-approved starting dose, maintenance dose, titration schedule, commercial injection pen, commercial vial or prescribing label. Altimmune continues to describe pemvidutide as investigational.

Human clinical research dosing

MOMENTUM obesity trial. Major studied doses were 1.2 mg, 1.8 mg and 2.4 mg, each given once weekly. These were separate trial arms. They were not an approved 1.2 mg starting dose, 1.8 mg maintenance dose and 2.4 mg advanced dose.

IMPACT MASH trial. The Phase 2b IMPACT study used 1.2 mg once weekly and 1.8 mg once weekly against placebo.

RECLAIM alcohol use disorder trial. RECLAIM used 2.4 mg once weekly for 24 weeks. That dose was studied specifically in this Phase 2 AUD protocol and does not establish 2.4 mg as a universal pemvidutide dose.

The strongest agreement across sources is once-weekly subcutaneous administration. The separate clinical-trial doses do not form one universal escalation ladder.

Research-community dosing

Community pages often reuse the genuine trial doses and turn them into a generic ladder: "starting" 1.2 mg weekly, "standard" 1.8 mg weekly and "advanced" 2.4 mg weekly. The numbers are real human research doses. The starting, standard and advanced labels are community reinterpretations, not approved clinical categories. These doses are human trial dose examples, not a standard pemvidutide protocol.

Duration and cycling

Pemvidutide has been studied over periods including 24 weeks and 48 weeks, and the Phase 3 MASH program is designed as longer-term disease treatment. It is being developed as long-term disease therapy, not a short peptide cycle.

No receptor-reset cycle has been established. There is no evidence-based requirement for:

  • 8 weeks on / 4 weeks off
  • 12-week cycles
  • 5 days on / 2 days off
  • monthly washouts
  • GLP-1 receptor resets
  • glucagon receptor resets

Expected results timeline

Weight-loss research. Appetite and gastrointestinal effects may occur relatively early, but clinically meaningful weight loss is evaluated over months rather than days. By around 24 weeks, meaningful body-weight and liver-related changes are measurable in liver and metabolic trials. At 48 weeks, MOMENTUM reported mean weight loss of approximately 10.3% (1.2 mg), 11.2% (1.8 mg) and 15.6% (2.4 mg).

Liver research. Large liver-fat reductions have been observed between about 12 and 24 weeks. At 24 weeks, relative liver-fat reductions reached approximately 75 to 76% in the 1.8 mg and 2.4 mg groups of the MASLD extension study. The IMPACT Phase 2b program evaluated biopsy-based MASH outcomes at 24 weeks, with positive results, followed by longer-term metabolic and fibrosis-related outcomes reported at 48 weeks.

Community-reported timeline. Community material often describes early appetite suppression, early GI effects and progressive weight loss over weeks and months. Many of the percentages quoted online are copied from clinical trials, and community timelines are not controlled trial outcomes.

In clinical research, measurable weight and liver changes develop over months, with important trial endpoints assessed around 24 and 48 weeks.

What does pemvidutide target?

Pemvidutide activates GLP-1 receptors and glucagon receptors. The two pathways contribute different effects: GLP-1 primarily helps reduce appetite and food intake, while glucagon has important effects on the liver and energy metabolism. Together, the drug is intended to affect both energy intake and how stored fuel is handled metabolically. Verdict: well-established mechanism.

Does glucagon just raise blood sugar?

No. Glucagon can raise blood glucose, but glucagon-receptor signaling also affects liver metabolism, fatty-acid oxidation, lipid handling and energy expenditure. Altimmune's development rationale emphasizes direct liver effects from glucagon-receptor activation alongside appetite and weight effects mediated through GLP-1.

Human half-life

Pemvidutide is long acting enough for once-weekly administration, which is directly supported by the human clinical-development program. Dosing frequency follows the human clinical schedule rather than being reverse-engineered from a quoted half-life.

Does once-weekly dosing make sense?

Yes. This is one of the most secure conclusions for pemvidutide: the major clinical programs use once-weekly subcutaneous administration. Verdict: strongly evidence-supported.

Does twice-weekly dosing make sense?

There is no controlled evidence that splitting the weekly dose improves weight loss, MASH outcomes, liver-fat reduction or tolerability. Verdict: not established.

Does daily dosing make sense?

No major clinical pemvidutide program uses daily administration. Verdict: not established.

Morning or evening?

Neither has shown superiority. Verdict: preference, not an evidence-based requirement.

Does it need to be taken fasted?

No evidence shows fasting is required. Verdict: not established.

Pre-workout timing

Pemvidutide is a long-acting weekly metabolic drug. There is no evidence that taking it near a workout improves efficacy. Verdict: no demonstrated advantage.

Dose escalation and titration

The human program has used more than one dosing and titration design. Titration may be useful for tolerability, but there is no finalized universal pemvidutide titration system. Verdict: study-specific.

Is 1.2 mg the starting dose?

Not in the sense that an approved GLP-1 product has a labeled starting dose. 1.2 mg is a human clinical trial dose, not an FDA-established universal starting dose.

Is 1.8 mg the standard dose?

No. It is a well-studied clinical dose, especially in liver research, but pemvidutide has no FDA-approved maintenance dose.

Is 2.4 mg the "advanced dose"?

No official dose tier exists. 2.4 mg is a genuine human trial dose used in obesity and AUD research; calling it the "advanced dose" is community language. Verdict: human trial dose, not an official dose tier.

Does it need an 8 to 12 week cycle?

No. Clinical studies have lasted 24 weeks, 48 weeks and now longer registrational development, rather than short peptide cycles. Verdict: no evidence-based short-cycle requirement.

Does it need a receptor reset or washout?

No scheduled GLP-1 or glucagon-receptor reset has been demonstrated. There is no established 2-week reset, 4-week reset or monthly receptor recovery break as part of chronic pemvidutide treatment. Verdict: no demonstrated requirement.

Does the route match the research?

The major human trials used subcutaneous injection once weekly. Verdict: strong match to the human evidence.

Evidence

What is pemvidutide?

Pemvidutide (development name ALT-801) is an investigational, long-acting peptide drug that activates two hormone receptors: the GLP-1 receptor and the glucagon receptor. Altimmune describes it as a roughly balanced 1:1 GLP-1 / glucagon dual receptor agonist. It is being developed primarily for MASH, alcohol use disorder and alcohol-associated liver disease.

  • GLP-1 activity: can help reduce appetite, increase fullness, lower food intake and regulate blood sugar.
  • Glucagon activity: has important effects on liver metabolism, fat oxidation, liver fat, lipid handling and energy use.

Pemvidutide is designed to combine lower food intake with direct metabolic effects involving the liver. It is peptide-based and designed for weekly administration. It is not a GIP agonist, insulin, a stimulant or an approved medication.

How well studied is it?

Pemvidutide is well studied for an investigational drug, but it is not approved. It now has:

  • Phase 1 human research
  • a completed Phase 2 obesity program
  • human MASLD studies
  • a completed Phase 2b MASH trial
  • a completed Phase 2 alcohol-use-disorder trial with topline results
  • an ongoing Phase 2 alcohol-associated liver disease program
  • an active Phase 3 MASH program (Altimmune initiated the global PERFORMA Phase 3 MASH trial on August 3, 2026)

Weight loss

The Phase 2 MOMENTUM obesity program studied once-weekly subcutaneous pemvidutide at doses including 1.2 mg, 1.8 mg and 2.4 mg. At 48 weeks, sponsor-reported mean weight loss was approximately:

ArmMean weight loss at 48 weeks
1.2 mg10.3%
1.8 mg11.2%
2.4 mg15.6%
Placebo2.2%

More than 30% of participants receiving 2.4 mg lost at least 20% of baseline body weight in the reported MOMENTUM results. This is meaningful human evidence, but pemvidutide is still investigational and is not an FDA-approved obesity treatment. MOMENTUM supports weekly SC dosing, substantial weight loss and genuine human dose-response research. It does not establish an FDA-approved obesity dose or a universal 1.2 to 1.8 to 2.4 mg escalation.

MASH

The IMPACT Phase 2b trial was designed to assess MASH resolution and fibrosis outcomes with pemvidutide versus placebo. Altimmune reported that the 24-week data showed statistically significant MASH resolution without worsening of fibrosis, and later reported continued improvements in noninvasive fibrosis-related measures through 48 weeks.

Pemvidutide has shown significant MASH activity and encouraging fibrosis-related signals in Phase 2b research. It has not been proven to reverse fibrosis. Phase 3 is now underway to determine whether these findings translate into a registrational treatment benefit.

Liver fat reduction

Human MASLD research has shown large reductions in liver fat. At 24 weeks, relative liver-fat reductions were approximately:

ArmRelative liver-fat reduction at 24 weeks
1.2 mg56.3%
1.8 mg75.2%
2.4 mg76.4%
Placebo14.0%

This is one of the strongest and most consistent findings in the pemvidutide program. It supports liver-fat reduction; it does not make pemvidutide a "liver detox" drug.

Alcohol use disorder

RECLAIM was a Phase 2 trial of approximately 100 adults with alcohol use disorder who received 2.4 mg pemvidutide once weekly or placebo for 24 weeks. Altimmune announced positive topline results on July 28, 2026, reporting a statistically significant reduction in heavy-drinking days (the primary endpoint), along with improvements in important secondary drinking outcomes.

These are sponsor-reported Phase 2 topline results, not a fully peer-reviewed efficacy paper. They are promising human interventional evidence; pemvidutide is not an approved AUD treatment.

What has it not been proven to help with?

Pemvidutide has not been established as an approved treatment for:

  • obesity
  • ordinary weight loss
  • type 2 diabetes
  • bodybuilding
  • muscle growth
  • athletic performance
  • spot fat loss
  • GLP-1-related fatigue
  • alcohol use disorder
  • alcohol-associated liver disease
  • general "liver detox"
  • longevity
  • anti-aging

It has also not been shown to selectively burn abdominal fat, increase energy, repair injuries, prevent weight regain after stopping, or give identical results at every dose. Clinical research may support some of these disease areas in the future, but clinical development is not the same as regulatory approval.

What studies have found

MOMENTUM (Phase 2 obesity). At 48 weeks: 1.2 mg, 10.3% mean weight loss; 1.8 mg, 11.2%; 2.4 mg, 15.6%; placebo, 2.2%.

MASLD research. At 24 weeks, relative liver-fat reductions of 56.3% (1.2 mg), 75.2% (1.8 mg) and 76.4% (2.4 mg) versus 14.0% with placebo.

IMPACT (Phase 2b MASH). Significant MASH resolution without worsening fibrosis, with additional fibrosis-related improvements over longer follow-up. This was strong enough MASH activity to justify registrational Phase 3 development. It does not show that pemvidutide reverses fibrosis in everyone.

RECLAIM (Phase 2 AUD). 2.4 mg once weekly for 24 weeks in approximately 100 participants. Altimmune reported significant improvement in the primary heavy-drinking-day endpoint in July 2026. Full peer-reviewed efficacy publication is still needed.

PERFORMA (Phase 3 MASH). Altimmune initiated the global PERFORMA Phase 3 MASH program on August 3, 2026. Pemvidutide is a late-stage investigational drug rather than an early experimental peptide.

Regulatory status

StatusDetail
FDA approvedNo. Pemvidutide remains investigational.
Fast TrackYes, for MASH and for alcohol use disorder
Breakthrough Therapy DesignationYes, for MASH, granted January 5, 2026
Phase 3Underway: PERFORMA began on August 3, 2026

Breakthrough Therapy Designation is not FDA approval. It is intended to expedite development and review when preliminary clinical evidence suggests substantial potential benefit. Pemvidutide is a Phase 3 investigational drug for MASH, and it remains unapproved.

Three pemvidutide contexts

1. Clinical-trial pemvidutide. Altimmune's controlled investigational drug, with controlled manufacturing, a defined formulation, route and dose, and clinical monitoring.

2. Human clinical-trial doses. 1.2 mg, 1.8 mg and 2.4 mg are genuine human research doses. They are trial doses, not FDA-approved dosing instructions.

3. Community research pemvidutide. Online research products may differ in manufacturing, formulation, excipients, sterility, purity, concentration and handling. A vial labeled "Pemvidutide 10 mg" cannot be assumed to match the investigational product used in Altimmune trials.

Bottom line

Pemvidutide has much stronger human evidence than most compounds sold as "research peptides," but it remains an investigational drug. The strongest dosing evidence supports once-weekly subcutaneous administration. Major human trial doses include 1.2 mg, 1.8 mg and 2.4 mg; these are clinical-trial doses, not an approved starting, standard and advanced dose ladder. Phase 2 research reported up to approximately 15.6% mean weight loss at 48 weeks. For liver disease, pemvidutide has produced large liver-fat reductions and significant MASH-resolution activity and has progressed into Phase 3 MASH development. For alcohol use disorder, positive Phase 2 topline results were announced in July 2026, but this remains an investigational indication.

Storage

There is no FDA-approved commercial pemvidutide formulation, so there is no approved reference product that defines a universal vial strength, concentration, diluent, bacteriostatic-water use, reconstitution method, post-reconstitution storage, refrigerated storage period, or 14-day or 28-day use period.

Community research products may provide their own instructions for bacteriostatic water, refrigeration and post-mixing use periods. Those are product or community handling instructions, not validated clinical pemvidutide stability data. Generic 14 to 28 day research-vial storage rules did not come from the clinical program.

Handling and stability information applies only to the exact formulation it was generated for. A research vial sold online is not automatically equivalent to the investigational clinical-trial product.

Contraindications & cautions

Formal contraindications

None are established, because no FDA-approved pemvidutide label exists. Contraindications listed for Wegovy, Saxenda, Zepbound or other GLP-1 drugs are not official pemvidutide contraindications.

Important cautions

  • Pregnancy and breastfeeding: human safety has not been established. Important investigational-use caution.
  • Significant gastrointestinal disease: pemvidutide causes GI adverse effects in human trials. Clinically relevant caution, not an FDA-established contraindication.
  • Other GLP-1 drugs: combining pemvidutide with another GLP-1 receptor agonist creates major pathway overlap. Unvalidated combination.
  • Pancreatitis history: may be clinically relevant when evaluating an incretin-based investigational drug, but pemvidutide has no approved label defining it as an official contraindication. Clinical caution, not a formal contraindication.
  • Medullary thyroid carcinoma / MEN2: the MTC / MEN2 contraindications on approved GLP-1 drug labels do not automatically apply to pemvidutide. No FDA-approved pemvidutide label currently establishes them as formal contraindications.

Side effects

Common human-trial effects

The most common adverse effects in the clinical program have been gastrointestinal: nausea, vomiting, diarrhea and constipation. Most have been described as mild to moderate in major trials.

MASH program

Altimmune reported a favorable overall tolerability profile through 48 weeks in IMPACT, including low treatment-related discontinuation. This is meaningful trial safety evidence, but it is not equivalent to years of postmarketing safety data from an approved drug.

RECLAIM AUD trial

RECLAIM also reported gastrointestinal effects such as nausea, vomiting, diarrhea and constipation in the 2.4 mg group. The topline results remain sponsor-reported pending fuller publication. An isolated serious event in a single trial is an individual trial event, not evidence of a common pemvidutide adverse effect.

Serious events

Serious drug-related events have not dominated the clinical program. Pemvidutide still lacks the long-term postmarketing safety history of an approved drug.

Compatibility

CombinationPathway logicVerdict
Pemvidutide + semaglutideBoth activate GLP-1 receptors.Major pathway overlap; no validated combination.
Pemvidutide + tirzepatideTirzepatide activates GLP-1 and GIP; pemvidutide activates GLP-1 and glucagon.Major GLP-1 overlap; combination unvalidated.
Pemvidutide + retatrutideRetatrutide activates GLP-1, GIP and glucagon, overlapping both main pemvidutide targets.Very high pathway redundancy; no validated combination.
Pemvidutide + cagrilintideCagrilintide acts mainly through amylin-related pathways.Less direct receptor overlap, but no validated combination protocol.
Pemvidutide + metforminDifferent primary mechanisms.Mechanistically nonredundant; combination claims require actual clinical evidence.
